Reading the Answers
When a client submits a form you are notified, with their name and the form they completed. Clicking the notification takes you straight to it.
You can also find it on the client’s profile, under the Forms tab.
The Submission
Open a completed form and you see the answers laid out beside the questions — exactly as they were asked, in the version of the form the client was actually sent. Editing the template afterwards does not change what you are looking at here.
At the top you see who filled it in and when it was submitted. Consent forms show the signature the client drew.
A form is frozen when you send it. If you later reword a question or add a new one, submissions already made keep the wording the client saw.
Exporting
Every submission exports as PDF or DOCX from the buttons at the bottom of the viewer. Useful for a signed consent you need to file elsewhere, or anything a client asks for a copy of.
Updating the Client Record
Forms ask for things you already keep on file: phone number, address, occupation, relationship status. Rather than retyping them, AyloCare offers them to you.
Open "Update client record"
It appears on a submission whenever an answer differs from what is on file. Answers that match what you already have are not offered — there is nothing to change.
Choose What to Copy Across
Each row shows what is on file now and what the client answered. Anything that would overwrite an existing value is marked Replaces an existing value, so you can see what you are about to lose.
Click "Update record"
Only the rows you ticked are written.
Nothing is written to the client record automatically. A form can be sent by a rule and completed without you touching it, but a change to the record itself always waits for you to confirm it.
Pulling Answers Into a Note
When writing a session note, use Insert answers from… to pull a completed form into it. The questions and answers are pasted into the note as text, where you can edit, trim or write around them like anything else you have typed.
This is a copy, not a link — editing the note does not change the submission, and the submission stays on the client’s record either way.
Only completed forms can be inserted. If the option is empty, the client has not submitted anything yet.
A Reminder About What This Is Not
AyloCare is not a monitoring or emergency service. Submissions notify you when they arrive, but they are not read, scored or checked for risk by anyone but you. Tell your clients how to reach emergency help.
