Forms & Intake
A form is a template you send to a client instead of writing yourself. They open a link, answer the questions on their phone, and the answers arrive on their record.
There is no account to create, nothing to install, and no PDF to print, sign, scan and email back.
What You Can Send
Anything in your template library with Send to clients turned on — see Managing Your Templates. AyloCare ships a ready-made library you can use as it is or edit:

Everything exists in both English and Greek, and your client is sent the version in their language.
How It Works
- You send a form — to one client from their profile, or to several at once from the Forms page
- They get a link — by email, or you copy it and send it however you like
- They confirm their date of birth and answer at their own pace; their progress saves as they go
- You read the answers on their record, and copy anything that belongs on file across to it

Two Things Worth Knowing Up Front
New link does exactly that — it issues a new link and the one your client already has stops working. If you only want to see what the form says, use Preview instead. It changes nothing.
A form link keeps working if your subscription lapses. This is deliberate: a client half-way through a consent form should not be locked out over your billing.
What Forms Are Not
AyloCare is not a monitoring or emergency service. A submission notifies you when it arrives, but it is not checked for risk and nobody is watching it on your behalf. Tell your clients how to reach emergency help.
There is also no scoring. AyloCare does not ship PHQ-9, GAD-7 or any other instrument that produces a number for a clinician to interpret. A form collects answers; reading them is your job.
